Any suggestions on further fixes?

This image was very blurry when i got it and I know we are not supposed to sharpen so I left it alone. I got rid of most of the junk and fixed a couple things but I am not sure if anything else should be done. I softened the background and blurred it a bit rather than cloning or painting a new one so it would not look too fake. Any suggestions on what i should (or should not) do next.

wow the whites look a bit blown out...but you have cleaned it up nicely..
without being able to sharpen or clarify not much seems to help.

You have done a beautiful job on the repairs! 
Thanks so much for preserving the original background, it looks so much better to repair the original one than replacing it with a new one.

Like Jesterjeni I also think that the whites are a little blown out, you could add a Shadows/Highlights Adjustment layer.  Bring the shadow slider back all the way to the left, we don't need it.  For the Highlights you can slide the Tonal Width slider to appr. 15%.  This way you also get a little more detail and a "sharper" appearance?
(you will need to smooth some of the backdrop as well afterwards)
